Raccoon City is sealed off after a biological catastrophe, its residents slowly transforming into zombies while the few unaffected citizens live in fear and despair. Alice joins a small band of elite soldiers enlisted to rescue the missing daughter of the scientist who created the mutating virus. As the infection spreads and resources run low, they must fight through the overrun streets and find a way out before Umbrella erases its experiment from the face of the earth.

Milla Jovovich leads the cast as Alice, with Sienna Guillory as Jill Valentine and Oded Fehr as Carlos Olivera. The film received a Saturn Award nomination for Best Makeup. Its trailer was downloaded 8.5 million times between November 2003 and May 2004. The production prepared an exclusive variant of the M134 weapon, and the building that explodes on screen is a real administrative center in Toronto.

- At 00:07:50, Milla Jovovich can be seen wearing some kind of blue jacket (on her right shoulder), and in the next shot she is in a white sheet.
- The film is based on the Resident Evil series of video games (1996 – present, Capcom).
- Natasha Henstridge could well have played the role of Jill Valentine, but was unable to accept the offer due to more tempting offers. In the end, the role went to Sienna Guillory. The actress stated that she based her character's mannerisms and walk on her impressions of the video game of the same name.
- "Nemesis" is the name of the third installment of the cult video game that inspired the work on this film. That was originally the title of the second part of "Resident Evil." A little later, the title was changed to the current one — "Resident Evil 2: Apocalypse," because in 2002 the film "Star Trek: Nemesis" was released.
- Jason Isaacs could have played the role of Dr. Birkin, but dropped out of the project.
